Torres had put Chelsea ahead in the first half before Ribery equalized for Bayern. Hazard put Chelsea ahead in the first half of extra time before Martinez' equalizer.
Chelsea played all of the 30 minutes extra time with 10-men after Ramirez was red-carded in the second half of the match.
Romelu Lukaku missed Chelsea's last penalty to ensure Bayern, who scored all theirs, won the penalties 5-4.
